Complex Cholesteatoma successfully removed at Latakia National Hospital

Latakia, July 25 ( SANA)Medical staff atLatakiaNational Hospital have successfully removed a complex cholesteatoma from a 65-year-old patient with a history of chronic ear discharge and hearing loss, utilizing the Canal Wall Up (CWU)surgical technique.

The procedure was performed despite complex anatomical anomalies, including a low mastoid tegmen, an anteriorly positioned sigmoid sinus, a high jugular bulb, and an exposed facial nerve, making it one of the most delicate otological surgeries.

The surgical teamfully excised the tumor while preserving vital structures, with no hemorrhage or neurological complications. The patient was discharged in good condition just six hours post-operation.

Dr. Manhal Khaddam, the lead specialist, explained that the convergence of these anomalies required meticulous dissection to avoid severe complications like facial nerve injury, meningeal damage, or venous bleeding.

He noted that while the CWU technique presented a restricted working space compared to the Canal Wall Down (CWD) approach, it successfully preserved the external auditory canal, middle ear volume, and the ear’s natural self-cleaning mechanism, yielding better hearing outcomes and avoiding issues linked to an open mastoid cavity.

Dr. Ivan Asaad, supervisor of ontological operations, confirmed the patient was discharged in stable condition with antibiotics, analgesics, and strict instructions to keep the ear dry.

He emphasized that the risk of recurrence remains low with rigorous follow-up, which includes microscopic ear examinations and audiometry at 6 to 12 weeks, followed by a Diffusion-Weighted MRI (Non-EPI DWI) at 12 to 18 months to detect any residual tissue, with a second-look surgery reserved if necessary.
