CCSS agilizará lectura de medio millón de estudios radiológicos en jornada extraordinaria

The Costa Rican Social Security Fund (CCSS) will speed up the reading of more than half a million radiological studies using exceptional payment modalities for results; This was announced this afternoon by its executive presidentMSc. Marta Eugenia Esquivel Rodríguez.
As explained by the head of the CCSS, “this constitutes a significant effort to improve medical care and reduce waiting lists and marks a milestone in the implementation of payment-by-results modalities with the objective of increasing the efficiency and quality of medical services offered to the Costa Rican population.”
The studies include nearly 250,000 general ultrasounds; as well as a similar amount between mammograms and tomography scans. These diagnostic tests allow health personnel to examine the inside of the body in detail without the need for invasive procedures and, based on the results, determine and plan the therapeutic options available to a patient.
The official added that, in the case of reading CT scans, nearly 55,000 will be included and those patients with oncological conditions and other medical emergencies will be prioritized, so that the most vulnerable patients receive the care they require in a timely manner, potentially reducing the adverse impact on their health. This also represents a step forward in the fight against long waiting lists and guarantees faster and more effective care for patients with critical needs.
The reading of these tests will be carried out on an extraordinary day and health personnel will be paid using a methodology related to payment by results and human resources management in the health sector, called: Maximum Unit Cost of Coverage for Health Care based on the Gross Domestic Product (CUMCAS-PIB).
Precisely, this methodology seeks to improve efficiency and effectiveness in the provision of health services, particularly with regard to timely patient care and the management of waiting lists. In addition, it constitutes an institutional effort to define a payment-by-results model based on macroeconomic indicators, indicators from the Organization for Economic Cooperation and Development (OECD) and specific indicators from the CCSS.
The strategy will be under the supervision of the Waiting List Technical Unit (UTLE) and includes measures to guarantee the quality and accuracy in the reading of the studies. For example, in the case of mammograms and tomography scans, a peer review system and the implementation of rigorous quality controls will be used.
“This supervision mechanism ensures that each patient receives an accurate and reliable evaluation, essential for the diagnosis and effective treatment of their pathology,” added Master Esquivel.
For her part, Dr. Verónica Quesada Espinoza, director of the UTLE, explained that the project will be directed from that unit, with the support and direct supervision of the Medical Management.
The doctor explained that the process will begin with the purification of the studies pending reading. At this stage it will be determined which of the pending studies meet the technical criteria to continue immediately to the reading and reporting stage. Additionally, a prioritization level will be given
individualized that will allow the case of each patient to be resolved individually.
The group of patients whose situation will be resolved includes insured people who, in some cases, have been awaiting a diagnostic response from the institution since 2016.

La Caja Costarricense de Seguro Social (CCSS) agilizará la lectura de más de medio millón de estudios radiológicos utilizando modalidades de pago excepcional por resultados; así lo anunció, esta tarde, su presidenta ejecutivaMSc. Marta Eugenia Esquivel Rodríguez. Según explicó la jerarca de la CCSS, “este constituye un esfuerzo significativo por mejorar la atención médica y reducir las listas de espera y marca un hito en la implementación de modalidades de pago por resultados con el objetivo de aumentar la eficiencia y la calidad de los servicios médicos ofrecidos a la población costarricense”. Los estudios incluyen cerca de 250 000 ultrasonidos generales; así como una cantidad similar entre mamografías y tomografías. Estas pruebas diagnósticas permiten al personal de salud examinar con detalle el interior del cuerpo sin necesidad de procedimientos invasivos y, a partir de los resultados, determinar y planificar las opciones terapéuticas con que cuenta un paciente. La funcionaria agregó que, en el caso de la lectura de tomografías, se incluirán cerca de 55 000 y se priorizará a aquellos pacientes con condiciones oncológicas y otras urgencias médicas, de manera que los pacientes más vulnerables reciban la atención que requieren de manera oportuna, reduciendo potencialmente el impacto adverso en su salud. Esto además representa un paso adelante en la lucha contra las prolongadas listas de espera y garantiza una atención más rápida y eficaz para los pacientes con necesidades críticas. La lectura de estas pruebas se realizará en jornada extraordinaria y al personal de salud se le cancelará utilizando una metodología relacionada con el pago por resultados y la gestión de recursos humanos en el sector de la salud, denominada: Costo Unitario Máximo de Cobertura para la Atención en Salud basado en el Producto Interno Bruto (CUMCAS-PIB). Precisamente, esta metodología busca mejorar la eficiencia y efectividad en la prestación de servicios de salud, particularmente en lo que respecta a la atención oportuna de pacientes y la gestión de listas de espera. Además, constituye un esfuerzo institucional para definir un modelo de pago por resultados basado en indicadores macroeconómicos, indicadores de la Organización para la Cooperación y el Desarrollo Económico (OCDE) e indicadores específicos de la CCSS. La estrategia estará bajo la supervisión de la Unidad Técnica de Listas de Espera (UTLE) e incluye medidas para garantizar la calidad y la precisión en la lectura de las estudios. Por ejemplo, en el caso de mamografías y tomografías se utilizará un sistema de revisión por pares y la implementación de controles de calidad rigurosos. “Este mecanismo de supervisión asegura que cada paciente reciba una evaluación precisa y fiable, fundamental para el diagnóstico y el tratamiento eficaz de su patología”, agregó la máster Esquivel. Por su parte, laDra. Verónica Quesada Espinoza, directora de la UTLE, explicó que el proyecto será dirigido desde esa unidad, con el apoyo y supervisión directa de la Gerencia Médica. La médico explicó que el proceso iniciará con la depuración de los estudios pendientes de lectura. En esta etapa se determinará cuáles de los estudios pendientes cumplen los criterios técnicos para continuar de forma inmediata a la etapa de lectura y reporte. Además, se otorgará un nivel de priorización individualizado que permitirá resolver de manera individualizada el caso de cada paciente. El grupo de pacientes a quienes se les resolverá su situación incluye a asegurados que, en algunos casos, desde 2016 están a la espera de una respuesta diagnóstica por parte de la institución. www.ccss.sa.cr | Blog de Noticias
